Do I Qualify for In-Home Visits? 

In order to comply with Medicare guidelines, there are certain criteria a patient must meet to be considered "homebound". These criteria include, but are not limited to:​

​

  • Patient requires use of an assistive device for ambulation such as a cane, wheelchair, walker, etc. 

​

  • Patient requires use of special transportation or relies on others for transportation.

​

  • Patient has a medical or cognitive condition that makes it unsafe to leave the home.

​

  • Leaving home requires a considerable and taxing effort.

​
